POSITION OVERVIEW 

The Pharmacist at Grace Clinics of Ohio is responsible for dispensing medications, providing medication counseling, and ensuring that patients receive the correct prescriptions in a safe and effective manner. This role involves reviewing patient medical histories, educating patients on proper medication use, and collaborating with the healthcare team to optimize treatment outcomes for underserved populations. 

KEY RESPONSIBILITIES

• Medication Dispensing: Accurately dispense medications based on prescriptions provided by healthcare providers, ensuring correct dosages and instructions. 

• Patient Counseling: Educate patients on proper medication usage, potential side effects, and interactions, ensuring they understand how to take their medications. 

• Medication Review: Review patient medication histories and prescriptions to identify any potential drug interactions, allergies, or contraindications. 

• Collaboration with Healthcare Providers: Work closely with physicians, nurses, and other healthcare professionals to discuss treatment plans, address medication concerns, and ensure comprehensive patient care. 

• Inventory Management: Manage medication inventory, ensure adequate stock levels, and oversee medication ordering, storage, and disposal in accordance with clinic policies. 

• Health Education: Provide general health and wellness education, including information on over-the-counter medications, supplements, and preventive care. 

• Record Keeping: Maintain accurate records of medications dispensed and patient interactions, adhering to clinic protocols and regulatory guidelines. 

QUALIFICATIONS

• Doctor of Pharmacy (Pharm.D.) degree from an accredited program. 

• Valid pharmacist license in the state of practice. 

• Strong knowledge of pharmacology, medication therapy management, and clinical practices. 

• Excellent communication skills, with the ability to explain complex information clearly to patients and healthcare providers. 

• Detail-oriented and highly organized. 

• Ability to work collaboratively in a healthcare team environment.
